Aetna® Medical

In-network medical benefits on the Aetna Select℠ plan

Aetna contracts with a large network of health care providers so that you can find in-network doctors and hospitals in your area. Start by visiting Costcobenefits.com and clicking “Find a Doctor” under Health & Wellness. You can search by name, provider type, condition or procedure. Look for the “Quality & Effective Care” label at the top of a doctor’s profile. It means the provider has met the high standards set by Aetna Smart Compare®.

  • Aetna Health Concierge is available for any questions Monday through Friday, 8 a.m. to 6 p.m. local time.
  •  The Aetna Health℠ app makes it easy to find in-network walk-in clinics, urgent care centers or emergency rooms when you need them, helping you avoid costly out-of-network rates. You can access your virtual ID card and find in-network dental providers, too.

Aetna Dental®

Dental benefits, including two free cleanings per year

Aetna Dental offers nationwide access to dental care. Your dental plan covers general dental care, specialist care and orthodontic care from in-network dental providers. To get the most from your plan, make sure to schedule your free routine dental cleanings and exams. And remember, with the Aetna Network Dental plan, you must use in-network dental providers, except for orthodontists and certain emergency care situations. Find in-network dental providers by contacting the Aetna Dental team or downloading the Aetna Health app. As long as you use an in-network dental provider, your dental plan covers these preventive services at 100% with no deductible:

  • Two routine exams and cleanings per year
  • Routine X-rays

Dental incentive

  • Earn a $50 credit toward your annual medical deductible or coinsurance when you get at least one annual dental cleaning
  • Available for you and each dependent enrolled in Aetna Medical and Dental® plans

Aetna Health Concierge

An Aetna Health Concierge can help you understand your medical and dental benefits. They can:
  • Answer claims questions
  • Help you understand your medical and dental benefits
  • Change the primary care physician (PCP) on your Aetna ID card

Live Healthy Team

Find one-on-one health support
The Aetna Live Healthy Team is a dedicated health resource for Costco employees and their enrolled family members. The team is your direct connection to a variety of health professionals, including:
  • Registered nurses to offer education, coordinate care and advocate for you on claims
  • Pharmacists to review, manage and provide education on your medications
  • Registered dietitians to offer guidance on nutrition and weight management
  • Behavioral health clinicians to navigate mental health resources, life events and stress
  • Social workers to find local resources for rent, food, transportation, caregiving and disability claims
The Live Healthy Team can provide support and services tailored to your needs and goals. You’ll work one-on-one with a Live Healthy Coach. Your coach will advocate on your behalf and connect you to the right care and benefits, including other Costco programs that may be available to you. 24-Hour Nurse Line

Talk with a registered nurse, day or night, 24/7
The 24-Hour Nurse Line is available 24/7 for your health-related questions. A registered nurse can help you decide where to go for care based on your needs or those of a family member. They can also help you find in-network urgent care, prepare for a doctor’s visit or answer general health questions.

EyeMed Vision

Vision and hearing aid benefits

EyeMed administers your vision and hearing aid benefits. The plan will pay up to $80 for your annual refractive eye exam and $175 toward the purchase of prescribed eyewear every year. You also get $1,750 toward the purchase of prescribed hearing aids every 4 years.

Costco Health Solutions (CHS)

Pharmacy benefits

Costco Health Solutions (CHS) provides the prescription benefits for your Costco medical plan. You can fill your prescriptions at any Costco Pharmacy or through Costco Home Delivery Pharmacy. When there’s no Costco Pharmacy available, you can go to any pharmacy that accepts your CHS card. For emergencies, you’ll be able to fill your prescription anywhere; however, you may have to pay the full cost and submit a request for reimbursement.

CHS Cares Medication Adherence Program

You can schedule an appointment with a CHS pharmacist who can:

  • Offer in-depth medication education and consultation
  • Discuss barriers to taking medication such as side effects or forgetfulness
  • Help you find ways to manage or simplify medications

Aetna Maternity Program

Maternity support and infertility benefits
The Aetna Maternity Program is available whether you’re planning a family, already pregnant or working through infertility. It offers expert guidance and support, including a fertility advocate. The program also includes access to a tobacco cessation program and tools to track your baby’s milestones.
Maternity incentive
  • Earn up to a $400 credit toward your annual medical deductible or coinsurance for you or your spouse or domestic partner when you enroll and engage in the Aetna Maternity Program during and after your pregnancy
  • The earlier in your pregnancy you enroll, the more you can earn
Costco medical plans do not cover charges related to a dependent child’s pregnancy, including labor and delivery, except services that qualify as preventive services under the Affordable Care Act.

98point6 offers on-demand text access to board-certified doctors 24/7. They can answer health questions, diagnose and treat urgent concerns such as colds or flu, and order prescriptions or lab work, all through the convenience of an app. Got a sick kid at home? Need quick non-emergency care? No appointments, waiting rooms or travel needed. Get help with:

Get free smart tools to monitor and track your progress. These programs are available at no cost to eligible employees and dependents who qualify.
  • Diabetes — Learn new ways to eat well, move more and manage stress to keep glucose levels in check. Includes a connected glucose meter and strips.
  • Hypertension — Take control of your blood pressure with guidance from your health coach and clinical specialist, as needed. Includes a blood pressure monitor and/or smart scale.

Omada for Joint & Muscle Health

Virtual physical therapy for joint and muscle pain

Omada for Joint & Muscle Health offers virtual one-on-one physical therapy with a licensed physical therapist to help you bounce back from an injury, build strength and flexibility, prepare for a new exercise program or stay pain-free during everyday activities. Pelvic health therapy includes pelvic exercises plus care before and after pregnancy. Note: Virtual physical therapy is not available to residents of Illinois.

FitMyFoot

Custom foot insoles for foot, knee or back pain

FitMyFoot offers custom foot insoles to all Costco employees and family members at a cost of $74.99 per pair. If you are suffering from foot, knee or back pain, custom foot insoles may help. No prescription is needed. Download the FitMyFoot app and scan your feet. Then visit the website to place your order. Your custom insoles will be shipped to your home address. Get up to 3 pairs of custom insoles per year covered by your Aetna medical plan. Deductible and coinsurance apply.

Sleepio

Online sleep improvement program

Sleepio is an online sleep improvement program that’s clinically proven to help you clear your mind, get better sleep and have better days in just six weeks. To get started, go to the Sleepio website and complete the coverage check. Then click the link in the confirmation email you receive to download the Sleepio app and create your account. Complete the baseline sleep quiz and start seeing the benefits of Sleepio.

Gennev

Virtual care for menopause
Gennev offers virtual one-on-one menopause care from board-certified Ob/Gyns and registered dietitians for 30-minute appointments. Get access to FDA-approved treatments like hormone replacement therapy (HRT), as well as support for nutrition, weight, sleep, mood and mindfulness.

Costco Tobacco & Nicotine Cessation Program

Quit tobacco and nicotine for good

The Costco Tobacco & Nicotine Cessation Program allows you to work one-on-one with a Costco pharmacist who will design a plan to help you quit tobacco and nicotine products. This may include access to over-the-counter nicotine replacement therapy at no cost. To learn more and enroll, go to Costcobenefits.com and click “Quit Tobacco” under Health & Wellness. RethinkCare is completely confidential, has no age restrictions, requires no diagnosis and is offered at no cost to all Costco employees and family members.

AbleTo helps you and your family members age 13+ enrolled in a Costco medical plan take charge of your emotional and mental health. From one-on-one teletherapy to digital activities, this no-cost virtual program offers support in managing:

Adoption Assistance

Receive adoption assistance from Costco
Benefits-eligible employees with one year of service have access to an adoption assistance benefit to reimburse them for qualified adoption expenses up to $5,000 per child (maximum of two adoptions) to help offset your adoption expenses. You can request reimbursement for certain out-of-pocket expenses:

  • Attorney fees
  • Immigration and translation fees

Unum – Basic Life & AD&D

Basic life and AD&D insurance
Unum administers your basic life and accidental death & dismemberment (AD&D) insurance. Life insurance is an important part of planning for your family’s financial stability in the event you aren’t here to help care for them. Basic life insurance is provided at no cost to all eligible employees enrolled in a Costco benefits plan, and to their family members enrolled in a Costco medical plan.

Unum – Supplemental Life & AD&D

Supplemental life and AD&D insurance
Unum administers your supplemental life and accidental death & dismemberment (AD&D) insurance. If you need more coverage than the basic life and AD&D insurance provided, you have the option to purchase supplemental life and AD&D insurance for yourself and your dependents.

Unum – Voluntary Short-Term Disability, Salary Continuation & Leaves

Help with income replacement when you become disabled or qualify for leave

Unum administers Costco’s voluntary short-term disability insurance plan (not available in all states), Salary Continuation, Costco Personal Medical Leave, the Family Medical Leave Act (FMLA) and Bonding Leave, which can be used while you bond with your child following birth or adoption. These programs help replace lost income if you become disabled due to an illness, non-work related injury or other medical condition such as pregnancy. Enrollment in a Costco benefits plan is not required.
Please note: If you live in a state with a paid family and medical leave program, your primary disability coverage is provided under your state’s program. You are also covered by voluntary short-term disability at a lower payroll deduction. Unum will reduce your STD benefit by any amount you are eligible to receive under the state benefit, even if you choose not to file a claim with your state.

Unum – Long-Term Disability

Long-term disability (LTD) benefits

Unum administers Costco’s long-term disability insurance plan, providing help with income replacement after you’ve been totally disabled due to a non-work-related injury or medical condition for 26 weeks. LTD insurance is provided automatically at no cost to employees enrolled in a Costco medical plan. As long as you don’t decline all Costco benefits, you can elect LTD insurance and pay a portion of the cost for coverage.

LifeMart is your employee discount program with savings available for gym memberships, virtual fitness and childcare.

Triple-S

Medical, dental, vision, hearing and prescription coverage through Triple-S

Triple-S medical coverage includes your dental, vision exam, hearing and prescription benefits. The plan will pay up to $80 for your annual refractive eye exam and $175 toward the purchase of prescribed eyewear every year.
